Four steps to your first PICM visit: intro call, enroll, new patient intake & a 60-minute first visit

Starting with a new doctor can feel like a lot, especially if you've spent years explaining your health history to people who didn't have time to listen. At Phoenix Integrative Concierge Medicine (PICM), we've designed the process to be simple & unhurried. Here's exactly what happens, from your first call to your first visit.

Getting Started: 4 Simple Steps

Most new members go from first call to first visit in 1–3 weeks, depending on scheduling availability.

  1. Reach out. Call us at (602) 905-7866 or send a message through our contact form.

  2. Have a brief intro call. This typically takes 15–20 minutes. We'll answer your questions & make sure the concierge model fits your needs.

  3. Enroll. If you decide to move forward, you'll complete & sign our Membership Agreement, Auto-Pay Authorization & Notice of Privacy Practices acknowledgment.

  4. Complete your intake form. You'll fill out our New Patient Intake form, and we'll schedule your first visit.

What to Bring

The more your physician knows up front, the more you'll get out of your first visit. Bring:

  • Medical records, especially recent lab results, imaging reports & specialist notes

  • A current medication list, including supplements & doses

  • Your specialists' names & contact information, so we can coordinate with them

  • Any test results related to your symptoms, like tilt table tests or lab work, if you've had them

  • A symptom log, if you keep one

  • Your questions. Write them down so nothing gets missed.

Your 60-Minute First Visit

Your first visit is a full hour, in person, with your physician. There's no rushing. It typically includes:

  • A thorough medical history, including past conditions, medications, family history & lifestyle

  • A physical exam focused on your concerns

  • A review of your symptoms & health goals

  • An initial care plan you'll build together

  • Time for every question you bring

If you're living with a complex condition like Ehlers-Danlos syndrome, POTS or MCAS, this is when we start piecing together the full picture, including what's already been tried & what your specialists have found.

After Your First Visit

As a member, you'll have ongoing access to your physician:

  • Same or next-business-day appointments, subject to availability

  • Two-way texting & secure portal messaging, monitored 7:00 AM–7:00 PM Arizona time, Monday through Friday

  • Extended follow-up visits, typically 30–60 minutes

  • Telehealth visits when appropriate

  • Care coordination with your specialists, including requesting reports & sharing updates
