Why an Annual Physical Matters More Than You Think

If you feel fine, it's easy to skip your yearly physical. You're busy, nothing hurts & the last one felt like a blood pressure check & a handshake.

That's fair. A rushed, check-the-box physical doesn't do much. But a good one, with a doctor who knows you, is one of the most useful hours you'll spend on your health all year. Here's what the research actually says & what makes the difference.

What the Research Says (Honestly)

Let's start with the inconvenient part. A large Cochrane review of 15 trials & more than 250,000 people found that generic health checks, offered to everyone the same way, had little or no effect on death rates from any cause, including heart disease & cancer. The reviewers also warned that routine checks can lead to unnecessary tests & treatments.

So a one-size-fits-all checkup isn't magic. What does help is more specific:

  • The right screenings at the right time, based on your age, history & risk

  • An ongoing relationship with the same doctor. A 2018 review in BMJ Open found that in 18 of 22 studies, greater continuity of care with the same doctor was linked to lower mortality. The evidence is observational, but it's consistent

The best annual physical combines both: personalized prevention, delivered by a doctor who actually knows you.

What a Good Annual Physical Covers

A real conversation

Your doctor should review your health history, medications, family history, sleep, stress & anything that's changed in the past year. This is where small problems get caught before they turn into big ones.

A physical exam

Vitals like blood pressure & weight, plus a focused exam based on your history & symptoms.

Screenings that fit you

The U.S. Preventive Services Task Force (USPSTF) recommends specific screenings for adults based on age & risk. A few examples:

Screening

Who it's recommended for

Blood pressure

Adults 18 & older

Depression

All adults

Anxiety

Adults 64 & younger

Hepatitis C

Adults 18 to 79

HIV

Ages 15 to 65

Colorectal cancer

Adults 45 to 75

Breast cancer (mammogram)

Women 40 to 74, every 2 years

Cervical cancer

Women 21 to 65, at set intervals

Prediabetes & type 2 diabetes

Adults 35 to 70 with overweight or obesity

Lung cancer

Adults 50 to 80 with a significant smoking history

Your doctor may recommend more or less based on your personal & family history.

A plan for the year ahead

You should leave knowing what's next: labs, referrals, follow-ups & what to watch for.

Why It Matters Even More With a Chronic Condition

If you're living with something like Ehlers-Danlos syndrome, POTS or MCAS, your appointments tend to focus on whatever is flaring right now. That's understandable, but it means the basics can slip through the cracks.

An annual physical is a chance to step back & look at the whole picture:

  • Catch up on routine screenings that got pushed aside

  • Review every medication & supplement for overlap or interactions

  • Check in on how each specialist's plan fits together

  • Set goals for the year instead of just reacting to symptoms

Complex conditions don't make you immune to everyday health issues. They just make those issues easier to miss.

Why a Rushed Physical Falls Short

In a traditional practice, a physical often has to fit into a short slot. That leaves little time for questions, family history or the "one more thing" you've been meaning to mention.

A physical is only as useful as the conversation behind it. If you leave without a plan, or without feeling heard, it's worth looking for a different setup.

Frequently Asked Questions

Do I need a physical every year if I'm healthy?

Not every test needs to happen every year. But a yearly visit keeps your screenings on schedule & your relationship with your doctor current, which is where the real value is.

What should I bring to my annual physical?

A list of your medications & supplements, any new symptoms or questions, recent records from specialists & any family health changes.

Is an annual physical the same as a sick visit?

No. A physical focuses on prevention & the big picture. A sick visit focuses on a specific problem.
